PLEASE NOTE: This form MUST be notarized!! We’ll have a notary prior to this meeting.

8:15 AM Sanctuary; After worship, depart for ASP!
Saturday, June 30th Return to Anderson Hills UMC

Sunday, July 8th Celebration Potluck Dinner

Requirements to Attend:
• Must have completed the eighth grade
• Must have completely filled out and returned ALL of your forms by the due dates requested
• Submission of your volunteer fees ($350.00) by the due dates requested.
Note: May use profits from sale of Little Ceasar’s Pizza kits and mulch sales.
• Must attend AT LEAST (2) of the (3) study/preparation sessions scheduled.
